An inverted "shark fin" ECG pattern: What is the diagnosis?

Abstract

A 62-year-old man presented with severe chest pain and hypotension. The ECG demonstrated deep ST-depression in leads V3-V5, fused with QRS complex and T wave, resembling an inverted "shark fin" pattern. We discuss the clinical significance of this unusual ECG pattern.
